Creatine: What It Actually Does and How to Take It

Everyone is talking about creatine right now. Huberman’s talking about it. Rhonda Patrick’s talking about it. Mel Robbins is talking about it. Your coworker is probably talking about it. And honestly, most of what you’re hearing is pretty good, but it’s spread across a dozen podcasts and hours of content you don’t have time to sit through.

So here’s what you actually need to know, without the noise.

What creatine is and what it does

Your body already makes creatine. Your liver and brain produce somewhere between one and three grams a day on their own. It gets stored in your muscles and brain as creatine phosphate and your body uses it to make energy, specifically the fast, explosive kind your muscles demand during hard training.

When you supplement with it, you’re topping off those stores. Fuller stores mean you can push a little harder, recover a little faster between sets, and over time train at a higher volume. That increased training volume is what actually drives more muscle. Creatine doesn’t build muscle directly the way protein does. It lets you do more work, and the extra work builds the muscle.

It also pulls water into muscle tissue. Some people interpret that as bloating or unwanted weight gain. It’s not. It’s your muscles holding onto more fluid, which is part of why creatine works.

The brain piece

This is where things have gotten interesting in the last few years and why creatine has blown up beyond the gym crowd.

Your brain is an extremely fast energy user. It burns through ATP constantly and the phosphocreatine system plays a real role in keeping that energy supply steady. Research has started showing that when the brain is under stress, whether that’s sleep deprivation, cognitive load, or aging, having more creatine available in brain tissue appears to help. There’s a frontal bias to where creatine seems to matter most in the brain, which is the area tied to working memory, focus, and strategic thinking.

The catch is this: your muscles are greedy. At a standard five-gram daily dose, your muscles absorb most of what you take in, and very little gets to the brain. Research out of Germany found that you need to get up to around ten grams per day before brain creatine levels measurably increase on MRI. That’s why you’re hearing people like Rhonda Patrick say they’ve moved from five to ten grams daily.

For acute situations like a night of poor sleep or heavy travel, some researchers have tested doses around 0.35 grams per kilogram of body weight, which comes out to 20 to 25 grams for most people. One study found that people in that sleep-deprived state weren’t just performing at their normal baseline after that dose, they were performing better than their rested baseline. That’s a notable finding.

All of this is early and some studies are small. But the trend line is consistent, and creatine has decades of safety data behind it, so the risk-reward makes sense.

For men

The basics apply here. Five grams a day of creatine monohydrate is the standard dose and it’s well supported. You’ll see your training volume go up, your recovery between sets improve, and your body composition shift over time, especially when combined with consistent strength training.

If you want the brain benefits, ten grams a day is where the more recent research points. You can split it, five in the morning and five later in the day works well and reduces the chance of any GI issues.

You don’t need to load. The original loading protocol, fifteen to twenty-five grams a day for a week, came out of research settings where scientists needed to saturate muscle stores quickly for short studies. If you take five grams a day consistently, you’ll be fully saturated in about three to four weeks. The loading phase just gets you there faster. It works, but it’s not necessary and it’s more likely to cause stomach issues.

You also don’t need to cycle off. There’s no evidence that washout periods are necessary or beneficial.

For women

The bloating concern comes up constantly and it’s worth addressing directly. Creatine pulls water into muscle tissue, which is the whole point. That’s not the same as looking puffy or gaining fat. Most women who notice true bloating from creatine are either taking flavored products loaded with fillers and flow agents, mixing creatine into a protein shake without enough liquid, or jumping straight to a high dose.

Dr. Stacy Sims, who has done a lot of the research on women and creatine specifically, recommends starting at 1.5 grams and working up gradually. Mix it in a big glass of plain water. Use unflavored creatine monohydrate with no additives. Give your body time to adjust.

The female-specific research is also catching up. Women tend to have lower baseline creatine stores than men, partly due to hormonal differences, which means they may actually see a more pronounced response to supplementation. Research has shown creatine benefits for women across the lifespan, with specific relevance around perimenopause and post-menopause. One study found that a third of post-menopausal women who supplemented with creatine alongside resistance training moved from mild cognitive impairment scores into the normal range. That’s not a small finding.

For women who are strength training consistently and not experiencing issues, three to five grams daily is a solid target. If cognitive support is the goal, moving toward ten grams daily, split into two doses, aligns with where the research is pointing.

How to buy it and take it

Get creatine monohydrate. It’s the most researched form, it’s cheap, and there’s no credible evidence that fancier forms work better. Buy it unflavored. Mix it in water. If you’re taking a higher dose, split it into two servings.

That’s it. You don’t need the grape juice. You don’t need a special timing window. You don’t need to cycle it. Just take it consistently.

The bottom line

Creatine is one of the most studied supplements in existence. The safety record is long and solid. The muscle and performance benefits are well-established. The cognitive and brain health research is newer but growing fast and pointing in a consistent direction.

If you’re training hard and not taking it, you’re leaving something on the table. If you’ve avoided it because of bloating concerns or because it felt like a “gym bro” supplement, the research has moved well past that.
